Not the writers. It is the sysadmin/user. If the software author is
aware of bad interaction it behooves them to mention it in the README.
Just as a pharmacist will mention such interactions for a prescription.
The ultimate responsiblity belongs with the end user.
Lawyers love it when people are eager to blame someone else for their
own ignorance.
